A Message From The Supervisor:
San Francisco Department of Public Health just released an appointment registration link for the City College (55 Frida Kahlo Way) mass vaccination site for San Francisco residents that are over the age of 65 years old. Click HERE to register for an appointment if you fit this criteria.
If you do not fit the age criteria and are not a San Francisco resident, you will be turned away at the site. In addition, please note:
1. The vaccine is completely FREE of charge to you. You will be asked for insurance during the scheduling
process. Insurance is not required to receive the vaccine (select 'No Insurance'). You will not receive a bill of
any kind. You will not be asked for a copay if you have insurance. Only insurers will be billed.
2. FIRST DOSE: This screen is for scheduling of your first vaccine dose only. You DO NOT need to log into MyChart to schedule your first dose. You can simply choose a time from those available below and provide your information. You will receive a MyChart Sign Up activation e-mail as you physically check in on the day of your vaccination appointment. You may sign up for MyChart and schedule your second dose visit at that time.
3. SECOND DOSE:  If you are attempting to schedule your second vaccine dose, please do not use the times below. You must proceed to first login to your MyChart account to schedule.
4. The COVID vaccine is currently available to priority individuals by invitation only.
Please do not make an appointment if you have not received a direct invitation or been instructed to do so by your employer. You will be asked to present your employee ID (or other verification) on the day of your appointment to verify your eligibility to receive the vaccine at this time.
5. Please provide as much of your demographic information as possible as it will expedite your check in on the
day of the appointment, ensure we are able to coordinate with you for the second dose of the vaccine, and automatically provide the vaccination administration records to your primary care provider.
Please note that the City College site is drive thru only and you will need to arrive at your appointment in a vehicle.Â
Although our City Hall Office is closed to the public due to the ongoing COVID-19 pandemic, we are still here to serve you. If you have any difficulty during this time, please do not hesitate to reach out. We are available by email: safaistaff@sfgov.org or by phone 415-554-6976 (leave a message and we will call you back). You can also check for updates on my Twitter, Instagram, and Facebook. Stay safe and healthy.
Sincerely,
Supervisor Ahsha SafaiÂ
